Blade.org End User and Customer Membership Form

This form should be used by any entity which wishes to join Blade.org and which certifies it is not an independent software vendor (ISV), independent hardware vendor (IHV) or reseller and otherwise receives no revenue from the sale of equipment or services related to the blade server industry. In all cases, the Blade.org Board of Directors shall make the final ruling on whether a member meets this criteria. For 2008, fees are waived for members which join Blade.org in this category.

I have read, understand and agree that the entity I represent will abide by all terms and conditions of the Blade.org Membership Agreement.

I have the right and authority to enter the entity I represent into a binding legal agreement.

I certify that the entity I represent is not an independent software vendor (ISV), independent hardware vendor (IHV), or reseller and otherwise receives no revenue from the sale of equipment or services related to the blade server industry.
